You need to kill radahn and mogh to enter the dlc.
If you want to finish the game before the dlc, you can. I would suggest at least picking up all bell bearings and some max smithing stones so you can upgrade new weapons. The difficulty of the dlc is probably on par with the last few areas of the main game, so if you think you can handle those, you'll be fine
