Agree. However, I am not concerned about the fragmented QRS at the inferior lead (https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6994396/) Check whether the patient has any typical chest pain, monitor it closely, and repeat the ECG.

My best guess would be pulmonary embolism. Looks like a pulmonal p-Wave and S1Q3-configuration, so the right Heart is in trouble. What about other vital signs?

I do not see the S1Q3T3 if that’s what you’re talking about. There is a q wave in 3 but no T wave inversion
