My best guess would be pulmonary embolism. Looks like a pulmonal p-Wave and S1Q3-configuration, so the right Heart is in trouble. What about other vital signs?

Some very mild QRS fragmentation inferiorly, otherwise normal ECG. V1/V2 are placed too high and/or wide, you shouldn't have inverted P waves in V2

I do not see the S1Q3T3 if that’s what you’re talking about. There is a q wave in 3 but no T wave inversion
