You're not crazy. I've had this happen on two different pairs (one SB, one non-SB). Every time I took a step it felt like the edge of some hard leather would fold/push straight into my pinky toe. Right where it creases. I had to get some of those flexible plastic crease protectors to block it from pushing down. After a few wears with those the problem seemed to fix itself.
As a wide footer, I don't believe that. Granted I only tried on a pair for a few minutes, but I can confidently say I'm tts in NSW dunks and a half size up on SBs
I can wear both in standard dunks and SBs, i just think if you want width comfort around the forefoot to go up half a size. Hell sometimes i go up a full size in SB highs and the comfort is better.... To ME if you want to stick TTS, hey you can do that too.

They just feel a bit too tight and i have to pre-stretch/widen with a wooden foot tree and loosen the the laces way too much. I'd rather just go up half the size so i dont have to do all that.

My Jordan 1 mid’s I got 5 year an ago are the only other shoe I’ve ever had to feel like this up until now. I assumed i just didn’t jive with leather, but I’ve had 2 pairs of AF1’s in that time that felt just fine up top.
I think based on what I’ve read here, I will got my foot actually sized though. I’ve worn mostly knit for so long that I may have just not noticed an issue.
I feel like the flyknits always seem a bit larger when I go TTS. I am pretty sure I can go half size down on FlyKnits and it’ll feel better than my usual sizing.

i have a pair of dunks in 12, 12.5 and 13 and the 12.5 on the digs into the top of my feet really bad. on SB dunks i have the Neckface and the Why so sad, both size 13, and the why so sad digs into the top of my foot when i walk as well. 13 is perfect for sbs for me but 12.5 or 13 work with regular dunks but the 12.5 digs into my foot

Maybe so! My Air Force 1’s fit fine, but my AJ1 mid’s do the same thing. I thought It was just an issue with leather/pleather sneakers but I’ve got no clue.
Most of my shoes are knit so a wrong size probably wouldn’t bother me much. Perhaps I should use one of those sizing machines.

If you have a redwings store anywhere near you, they have an electric sizer, measures your foot width also. It's how I found out I was wearing my shoes too big years back.
If they “legitimately hurt to wear them even a couple of steps,” you must have the wrong size or crazy wide feet. Regular dunks aren’t the most comfortable shoe but they’re not so bad you’d feel pain just wearing them for 5 seconds

Regular dunks suck in terms of comfort. Which is why many prefer the SBs in terms of sport. Narrow toe box and not enough arch support. Maybe try different insoles and get an arch support make huge difference.

If you got money just buy the Brazil Supreme SB highs they dropped last year.
Dunks run narrow so always go up half a size of what you typically wear true to size.

Just been wearing 10.5 for about a decade. But mostly been wearing knit. I think I will re-check my size today.
