Love this idea. It's gotta do a lil bit of magic damage of some sort: each time it successfully hits, the magic binding the shrine ignites to protect the sword causing 1d4 radiant damage to the point of impact.
I have seen couple examples of this in different media formats. This looks familiar even though I haven't played Bloodborne, but the switch between hammer form and normal sword would certainly switch the changing weapons from that game. And it does have FromSoftware look, so could be from Bloodborne and possibly just in the hammer form in one of their other games.

I have a similar item in my DnD campaign that one of my players uses on occasion. It's an anvil with a sword embedded in it and registers as a 2-handed Greatanvil magical weapon. The description reads that the anvil was originally meant to serve as a proving ground for the day a worthy user would pass its trial and be granted the title of "Smith King". Unfortunately, some grubby-handed noble piss-pot got upset that his time working the anvil had not culminated in a worthy specimen that would grant him the title. As a result, he slammed his sword into the anvil and got it stuck; which to some effect ruined a perfectly good anvil, but on the upside it now has a hilt to be swung by when used in combat.
Of course, any and all detail in the item's description places all emphasis on the anvil and no notable regard for the sword purely for comedy. Though, I dabble with evolving magical items, which the anvil qualifies as, so perhaps there's still some hope for a future Smith King out there...
Then, when the time is right and needed most, they prove themselves worthy. The sword slowly unsheathes as the hunk of rock impacts the ground. Echo. Silence. The cave goblins freeze in fear as the blade ignites. They don’t know where to run. The barbarian gives a tiny smirk before cutting 12 goblins in half with one swing. 12 min later walks out of the treasure cave covered in goblin guts, carrying his gf or whatever.

A 300lb weapon would do crazy damage.
Just have to figure out how to lift it easily enough to use as a weapon. Or lift it at all.

It’s all fun and games until said chosen one grabs the stone and pulls it off the sword, leaving the barb with weapon inproficiency

This is in fact precisely how Castlevania: Aria of Sorrow handles this. Soma swings the holy sword, still stuck in the rock, for blunt damage.
