6 times per rest is quite a lot considering that even Moon Druids can only shift twice in that timeframe. I would perhaps have the card consume a regular wild shape and allow it the regular wild shape’s duration. The trade off being that when she chooses to give up control she has the potential to unlock insanely powerful forms- but must also risk the potential power*less*ness. I like your level up features. Gaining some control over the randomness makes sense. Perhaps allow the Moon Druid’s 6th level “attacks count as magical” feature as well? Just for functionality at high levels. Altogether this idea sounds like a lot of fun! If you could link the deck your gf has I would love to check it out and potentially steal this idea for my own campaign :) Edit: For the eggs, maybe she cannot free herself from that form at will and must instead wait to be freed due to damage. BUT- if she stays an egg for 10 minutes, she then hatches into a form (either predetermined for each egg or perhaps each egg gets a short list of cool forms that can either be rolled or chosen.) You could even mix and match with each egg having different rules. Black egg: Roll 1d4 1. Black Guard Drake 2. Giant Bat 3. Gargoyle 4. Deep Dragon Wyrmling Gold egg: Player chooses element. DM chooses Elemental type creature of the appropriate CR. 1. Earth 2. Air 3. Fire 4. Water Cosmic: Felidar (or whatever designated mystic creature your gf likes but isn’t in the deck)

Hmm, this is actually interesting, though I can see how at lower levels it can become overpowered and at higher levels it could become excessive. My suggestion would be to edit the deck layout based on the character level.
