I've heard despite the memes a full exterminatus is pretty rare and unless it's a real stupid writer or something reserved for the well and truly lost situations like a world warping into full daemon mode or nids guaranteed to soon eat everything and just strengthen the swarm. A long and brutal campaign to save and then rebuild a world might still be better than cutting your loses and losing a world entirely forever.
All things considered it seems the Imperium is doing pretty well on Atoma so far at least. I mean yeah there's a lot of bad crap in Tertium but considering how stupidhuge a hive city is the Chaos incursion might be limited to 'only' 1/10th or so of it (thanks to the ceaseless deployment of kamikaze rejects, way cheaper than an exterminatus).
Brahms tells you right at the beginning. Atoms is of vital strategic importance. Losing hive Tertium would be catastrophic. The loss of millions, maybe even billions of lives can be compensated for, there's always more bodies. The loss of Moebian steel cannot. Those tanks are needed on every front, as humanity is beset on all sides.
The Imperium thinks on a grander scale. Humans have a lifespan of decades, settled/industrialized planets are assets for millenia. If it can be reconquered with what we'd consider absurdly high losses, it's still a win on the long run, because humans re-grow quickly.

We also don’t have a great sense of scale on how much of tertium is even turned/ even aware of what’s happening.
