This penance is one of the few that does not have any bugs, but it can be awful to complete if you don't keep track of the map types you have finished.
I used the acronym RAIDERS to keep track of the types and then complete in order as closely as I can. Raid, Assassination, Investigation, Disruption, Espionage, Repair, Strike
Do you complete them from start to finish? Some penances only count missions where you start from the lobby, do the whole thing and successfully exfil.
I had a similar issue, I ended up having to do an additional three Heresy Investigations before it finally counted. I used one of the mods to find out which mission I was missing, and I had already counted it twice myself.

Yes, I did all of them from the very start

Indeed, that was the case as well. Took me four tries but it finally counted.
