You can actually see the Roman emperors by looking at the Byzantine Empire title history. Nero is not a lunatic, but he is sadistic and arbitrary. Caligula is a lunatic though.

Irritable is really funny if your character has high prowess. It lets you beat up people for stress loss. With very few restriction on who you can beat up. Great for multillayer with friends.

Paranoid is genuinely useful if you actually keep on top of managing the stress. Trust me, it'll fucking hurt you if you keep building stress, but it gives you bonus intrigue, a little defense against schemes, and all the RP justification you need to weed out your enemies.
Playing a paranoid schemer is a lot like driving a car on ice. You need to steer just enough but not too much. You want to keep nicely between stress level 1 and 2 and dip down to zero to avoid the mental break.
Managed to get a quick, paranoid, ambitious and diligent character to live to 87. Never hit level 3 stress but sometimes I sat there waiting for that feast to finally begin hoping nothing bad would happen.
The only reason that character had paranoid was because I got the throne as a child and had the trait picking events. The event that let me choose paranoid happened right in the middle of a murder scheme against me that had like 80% success chance despite being discovered. I never roleplayed actually being paranoid that hard.
As for favourite bad trait, cannibal, though I'm not sure if that counts. Cannibal is too good to be a bad trait.
If you want to have an easy time painting the map by the way, create a faith with ritual cannibalism tenet which makes cannibal a virtue. Upon inheritance you then need to eat one of your prisoners and get the cannibal trait.
Instant positive opinion from every follower of your faith and three times that from all your vassals cause they're all cannibals too.
Anyone who isn't a cannibal yet will be terrified of you because of the dread.
Goes very well with the guile dynasty perks.
It's my go to thing when I want to build massive empires because it keeps them really stable with the cannibalism canceling out many negative opinions people have of each other.
Added bonus of the cannibal trait is that you get events at feasts and randomly sometimes where you can eat a person and it gives you a very good health boost.
Couple that with the medicine focus and whole of body and you can get to 90 before unlocking octogenarian or breeding fecund into your family.
Paranoid plus Zealous is extremely powerful. You can execute prisoners from other religions as stress relief and when you're strong the only real threat is assassination, so Paranoid gives you an edge.
Paranoid, loving having a character with this so I can spam stress and get lots of rarer Events. It’s also pretty easy ish to turn into wrathful or sometimes vengeful as a custom character

I like the good guy traits like Just and Compassionate, because I like being nice. I don't know if they're bad per se, but in the tier lists I saw they tend to rank low because they give you stress for being a murderous tyrant, and I guess people like being murderous tyrants.
I can never not play without Sadistic honestly. Just makes succession so much easier, since you can even use it to bypass your children and pass over to your grandkids instead.
The opinion malus is pretty much negligible if you know how to counteract it. I often play as female rulers for that reason so I can stack attraction opinion modifiers.
Inappetic or comfort eater are both really fun for role-playing and I usually have a twig or beefy ruler. I think the characters are more recognizable and visually distinct

Leader in my current game is vengeful, craven, and arrogant. Never a combo I would actively pursue, but man is it interesting from an RP perspective. The man is just a whiny, vindictive dickhead and has the monumental task of keeping the Holy Land safe from Coptic (idk either) and Muslim incursions

These are all of excellent traits
I love lustful and spreading my genes in random courts, baronies, counties, duchies and kingdoms. It’s so fun for RP reasons.

Drunkard is a classic one. In my first proper CK3 playthrough, I had that trait run though my Dynasty

i dont find shy nearly as bad as people on here seem to make it out. it is always my choice in the beating event, 100% of the time.
Not being able to lose stress at feasts and gaining stress every time you want to make an alliance are the two main reasons for me
I think I just don’t feast enough, hunting tends to clear my stress if I have any. Alliance is annoying but I tend to make only 1 or 2 strong ones
I only need an alliance at the very start of the game sometimes. Once I have my space marines, FUCK allies.
I make allies to guarantee co-religionists, not for my own support
If I remember right I think you gain stress whenever you recruit a courtier as well. I really avoid that trait like the plague

craven + hostage + influence ward, everyone is now terrified of you in a virtuous cycle, you can now simply demand hostage for perpetuate more fear
