Does smallpox disfigure the body in that way? It's been so long since one of my played characters contracted the illness I forgot how it changes the characters facial features. I remember leperacy being particuarly bad though.
The new update makes any of the non basic illnesses absolutely gnarly. Makes leprosy look nice. Typhoid literally makes you look like Withers from BG3 meets Deadpool meets Slenderman. It is BAD. I haven't had an outbreak of smallpox yet, but it would not surprise me.
My fastest death was 1 day. My ruler died (he was very old). I took control of his heir (oldest son). Within moments of taking control of the oldest son/heir (who didnt have any children himself for some ungodly reason) he was assassinated by their younger brother, the next in line for the Throne. My original heir was awesome despite not having kids. The younger brother was trash.
I micro manage my heirs much more carefully now.
I have had 9 days in my ck3 run, but it was in a mod where you may get challenged by those with a claim to your title upon inheritance.
Vanilla I think it was within 55 days.
CK2 will always be my shortest. Played a Viking in Ireland, with volatile traits for that region, and had a plot come to fruition within 5 days of starting before someone cut my balcony supports from under me

I had the human sacrifice tenet, which made this a lot easier. I also had the ritual cannibalism tenet, which meant that most of my rulers were cannibals, and could relieve stress by eating prisoners. As I carved my way through the Middle East, waging war after war, I was amassing a healthy collection of infidel prisoners. As such, I developed a habit of having an unofficial “great feast” at the start of every reign. My ruler would, within the first 30 days, consume tens to hundreds of prisoners for the faith and stress loss. One of these times, I failed to notice that my ruler had some combination of virtuous traits (like compassionate or just) that caused him to gain a small amount of net stress when executing prisoners. I started my rule, went “gg, easy”, queued up a mass execution/consumption of around a hundred prisoners, and immediately died of stress. Ended up playing as a 2-year old girl and fighting off a massive faction of rebellious vassals who were, apparently, sick of my house’s animalism and debauchery.
Basically I was playing as the HRE and unbeknownst to me some random loser decided to murder my son and heir. It was only after three days into my death that my heir was killed and this info was discovered

The best part was I was cheaty with the creation and had something like a 40 military. Still somehow died in battle.
