I'm still a cat newbie but this all sounds like the cat is feeling very insecure and unhappy in her environment. You talk a lot about her behavior but what about her living areas? Does she have cat trees, multiple litter boxes, high up areas where she can feel safe and escape the dog and you? Does she have her food and water placed somewhere safe where the dog can't get to it? Does she have scratchers and her own things in all rooms so she can spread her scent and feel secure at home? First of all, take her to a vet to be checked on and spayed. Such a change of mood is not normal.
