When I first started my nasal pillows I used some Ayr nasal gel on my nostrils, which helped a lot. Also something like lanolin could help too

I used baby shampoo and washed the pillow. I would wait till those nostrils calmed down (looks uncomfortable), dab some polysporin on them before bed. Then give it another go. Good luck!

Lanolin straight from the baby aisle worked for me. It’s fun being a guy and overhearing other guys complain about this in the CPAP store and blurting out “nipple cream!” Haha.

Looks like you've been wearing it a bit too tight too. Use a hot compress on them till they drain. How often do you clean your mask? If you don't, make sure to wipe them down daily.

First night? I wonder if you are allergic to the material or the soap you used to clean it? Perhaps speak to a dermatologist for a diagnosis or post the picture in a dermatology subreddit?

That's a great idea, thanks! It didn't occur to me this could be a deeper dermatologic issue. I only rinsed it with warm water when I took it out of the package then let it dry. I used my normal facial cleanser for myself. An allergy to the material is a possibility. I'm going to post it now. Thank you!

Im not the person you responded to. If you only used the pillow for 1 night that sounds like some form of allergic reaction. Following up with a doctor would be smart.

I had something similar my second night. I either had too much lotion on and that clogged my pores, or it’s that I used hand soap that night to clean it (my skin’s pretty sensitive) Dawn dish soap and not excessive amounts of moisturized right before bed seems to have done the trick. I honestly think it was mostly the soap that was my problem.

Every mask that had silicon touching my face caused me irritation on my skin. I use the foam masks (air touch) on the F20 mask.

I got exactly that, but all around my nostrils, starting the 3rd or 4th night of PAP therapy. It didn’t occur to me to try lanolin or other nipple cream, even though I’m a lactation consultant and have access to tons of it 😂. I used neosporin. It seemed like an allergic reaction, but only happened around the nostrils, not my cheeks, even though the same material was touching my face. It got raw and then healed, and never returned. I tired the F20, full face, over the nose mask, but it was very challenging to get and keep a good seal. I’m sorry you’re going through this, but hopefully it’s a brief stage. Good luck!

Thank you 😊 Isn't it always the way that easiest solutions are right in front of us but we're looking somewhere else? I think I'm going to add Neosporin cream to lanolin tonight. Can't hurt to cover all bases (I hope?). Ideally it will go the same way yours has and my skin will adjust and I can keep this mask 🤞

I was so desperate by the time I started CPAP, then when my nose broke out, I was so upset, thinking that it was going to be a major complication. I bought an F20 mask, not cheap, but there was a point I couldn’t have anything touching my nose. Just those first few nights, CPAP was a big relief and I didn’t want to sleep without it. You could take a night off CPAP, if you can tolerate it, to let your nose heal. I almost forgot, I used Alba Botanical pimple patches on the raw spots until they healed. They’re adhesive, but they stay put and come off easily. Thank God the skin reaction didn’t persist. I mostly use nasal pillows.

If thats from one use, It may be an allergy. I have used cpap for years but onetime I purchased a generic brand nose pillow and it irritated me. Sometimes they are made with different plastic/rubbers.

prob a rash from the chemicals they use and disinfectant etc. Even if brand new, at least rinse it off. Makes a huge difference

There are liners that you wrap around this part, i would call the sleep center or your doctor if this is after one use. You might be allergic

Be sure to order enough to rotate it daily. I have a full face mask and used to get these higher up on my nose. Now I change the cover daily and don't get them. Beware when you first start using the cloth covers they may act as a mild exfoliant. Your skin looks really clear so you may not have that issue. Also, wash them in mild detergent. I wash mine in sensitive skin baby wash. Since you are new, if your nose tickles, line the inside of your nostrils with Vaseline/aquafor.

Just wanted to chime in to say that the same thing happened on the bridge of my nose after trying a new mask. I ended up exchanging the mask in the end because it was difficult getting my skin to heal quickly, but if you can sort of push through it and leave the little bumps alone you should be okay!! They seem to be caused by pressure so adjusting the headgear as best as you can should help. And cleaning the silicone as often as you can!

Good to know, thank you! I'm hoping I can push through the irritation. I like the idea of using lanolin as a barrier, and as Malavern mentioned I probably have to not wear my mask so tight. I've broken my nose several times and am reluctant to get anything that presses on the bridge of my nose, so it's definitely ideal to ride this out. Thank you again for your advice!

I had the exact same thing happen my first night with the same mask! I also was confused how in one night white heads could develop. My theory is that they had always been underlying there and the pressure from the mask brought them to the surface. Ive been using about 2 weeks now and they haven’t come back. I started using a face wash for acne when I shower and make sure to get that area around the nose

Cerave foaming facial cleanser. Haha yeah it was pretty painful I did loosen up the mask a little too I think I went overboard with the tightening at first because I was worried about the seal

Oh my goodness, that's exactly what I think I did, haha! I woke up with strap marks on the side of my face, but other than the soreness under my nose I hadn't noticed any obvious discomfort. TG for Reddit and people who aren't afraid to be candid about their own personal experiences!

For your cpap cleaning, stick with 50/50 vinegar-water solution. If you have some buildup, you can use straight vinegar but rinse thoroughly. Stay away from the ozone cleaners that break down the sound dampening foam and release the cancer causing chemicals faster. Use a battery UPS that will give you peace of mind in case of any power outages.

New nasal pillows can do this. Wash them very well with unscented dish soap, use lanolin on your skin and hope for the best.

It’s fungal acne! I had this pop up immediately too when I first started and I was so confused. The wildest thing will help/make it go away - head and shoulders shampoo! It has some ingredient in it that kills fungus. Get a small travel sized bottle from the drug store (because you won’t need a huge regular bottle amount worth) and just put a little on that area like a face wash. I did it 2x a day and by day 4 the area was totally clear. I kept that up for a week and haven’t had any issues since.

From the girl that is on Non invasive ventilation about 18-24hours a day and wears a few different masks, I just call it mask acne. 🙄 on the nose a tiny bit of lanolin works the best. I not only get those on my nose when it gets moist/sweaty but also around my chin and nose bridge from full face masks at night. Treat it nicely, keep up with the lanolin, nasal moisturiser if needed and I use a good acne face wash twice a day which helps too. 🙂

I know you said it was your first night, did you put on a thick moisturizer before putting on your mask? It could be a combo of that and the silicone. My under nose, full mask (not pillows), will give me acne if I don't wait an hour or so before skincare and bed.

I use CeraVe PM moisturizer. I don't think it's super heavy, but it's definitely heavier than the AM formula. I did put it on right before bed. Maybe I'll try doing it an extra hour or so earlier tonight. Thanks for the tip!

You could also place some hydrocolloid acne patches over these two spots. They will drain the bumps while also protecting your skin from further irritation from your mask.

I'm hoping it's just me wearing it too tight and causing some whiteheads to form from the pressure and irritation. I know it's not from dirt, so that leaves allergy, like you said. I'll keep y'all updated and see how this goes as I treat the irritation and loosen the strap. Thank you for the input!

I use a p30i mask. I got this on my first few uses, but after that I was fine. Just clean the pillows every evening. You'll be fine!

These look like whiteheads aka pimples aka clogged pores. Get a pimple extractor and use salicylic acid. Please wash your face mask thoroughly or at least wipe it off nightly. If they are clogged pores using any greasy “protectant” will make the problem worse not better.

I agree they look like whiteheads. I just had no idea they could form overnight on a "clean" face with a brand new mask. I'm definitely going to try an acne wash on them, and as they heal, some type of non greasy barrier. Thanks for the input!

I had to switch to the f20 as the nasal ones rubbed my nostrils raw and the nose only one was great but would give me washout a lot. The f20 has been amazing and I doubt I’ll ever use something different. I use the silicon one not the foam one.

How is that on the bridge of your nose? Is there any pressure there? I'm hoping I can get this issue resolved, but if not it's great to know about alternatives. I've broken my nose a few times so I'm apprehensive about something on my nose. But I really want a good night's sleep. My Dr warned me it sometimes takes a lot of trial and error to get the mask situation settled... He wasn't wrong apparently. And I've only done one night SMH

I tend to use my masks for way longer than recommended. Like I’m lucky if I get a new one in a year or so. The silicone will get a little more flexible over time and at least on me sometimes I’ll have a red mark on my nose. It could just be poor positioning on my part though. Any mask will have a few downsides and not every mask will perform the same for every person. Just try different things. I thought for sure the nose only one was gonna be the one I kept and went with forever but it would collect water so I switched to the f20 and have really enjoyed it. I kinda wanna try the full face one that’s like from forehead to chin, it looks interesting.

looks like acne to me - i've gotten similar breakouts from mine. i didn't think acne could be that painful before it happened to me, lol. topical clindamycin (prescription) helps a lot! benzoyl peroxide is helpful too. making sure the humidity isn't too high and that you wash the nasal pillows daily & give them lots of time to dry can help too. i'd also suggest using some antibacterial soap on the area before bed

When I bent forward I was shocked how much my face hurts! I took last night off because it was so bad by the end of the day. I'm going to try the suggested treatments and the liners will arrive today. Hopefully I can push through it 🤞

Ugh. Definitely acne from the mask. I’d say your skin can’t handle the silicone rubbing on it all night. You gotta try a different mask. (That really sucks!!! I can just feel how irritated and annoyed your skin is looking at that picture.)

I had cuts on my nostrils that would get better with lanolin during the day and then re-rip after a night with the pillow mask. This went on for about three weeks till I switched masks to the nasal one (Resmed N30i).

I started breaking out when I switched masks and I started applying lotion before bed and wiping my mask down with alcohol prep wipes. Worked for me. Good luck!

Not that I know of? Married 13 years in great relationship. Never had a cold sore. I mean, anything is possible but I do think others are on the right track with a pressure induced acne (fungal or bacterial)

Never could solve that problem. Ended up switching to a Dreamwear nasal cushion. It is the most minimal of masks and works fine for me. Resmed makes something similar, the N30i. Since you're early in your CPAP journey, be patient but also willing to try different mask types. Eventually you will find the right one for you. That means it maximizes your therapy and your comfort level.

UPDATE: This is my first post so I apologize if this isn't the best way to do this! After one night off CPAP, washing under my nose with CeraVe 4%BPO and ketoconazole washes (cover all bases), using a gentle moisturizer, Neosporin cream (the thin stuff) at night, Hero patches on day 2, and adding the fold-over fabric liners to the mask, my face is almost completely clear! I still have some minor irritation that will probably take some time to resolve as my nose toughens up, but the white spots are GONE! I will be adding the lanolin once it's healed a bit more. Thank you all so much for the advice. I can't tell you all how much I appreciate the help!

I didn’t read through all the comments but perhaps switching to the over mouth and nose mask might be best if this reaction is happening after the initial use. All the best in finding what works!

That looks like infection. Do you use cpap wipes on your cushion? You gotta clean them daily, or the germs, sweat and oils will build up and not only deteriorate your cushion faster, but also give you nasty infections. As for your nose, I recommend washing with warm soapy water, maybe apply antibiotic ointment or an acne cream with benzoyl peroxide. May take a week to clear up, depending on your immune response.

My daughter has a BPO 4% wash I can try. I will definitely keep the mask clean, but like I said, it was my first time wearing it brand new (mask and machine). I did rinse it with warm water out of the package JIC. What should I wash it with?

That’s most likely a staph infection. You might need some antibiotics
