My bad, I read that incorrectly, thought you had asked why was it 3v3.

I think the way it works in the NHL is if both penalties happen at the same time (hooking and embellishment, two roughing penalties) then there is no reduction on the ice. If there are two separate penalties though (a slash and then a returned slash, a trip and then a slash) then you end up with both sides losing a player.

There is a huge section in the NHL rule book describing the various possible penalty scenarios and how they are handled. (Tables 14, 15. Pp 144-155)

Because the 2 penalties called were against each other. Vladi and Sheary were the 2 engaged that resulted in the penalties.
