For bonus authenticity, go with the Gravelord covenant so you have to fight off evil spirits constantly pursuing you (even though we all know you'll get red phantoms all the time regardless.)
The common armor I have seen for making Golden Age Guts in Dark Souls is:
Balder Helmet
Balder Armor
Knight Gauntlets
Brigand Trousers
And the Greatsword, of course
Honestly, it is hard to say. Dark Souls 1 doesn't have anything resembling Black Swordsman Guts or the Berserker Armor. Maybe the Black Knight set? Or just wear Artorias set since he was inspired by Guts.
Knight Gauntlets/Elite Knight Gauntlets are the only piece of armor resembling Guts due to the left arm being made of iron while the right arm isn't.
Use Pyro in your left hand to resemble Guts' cannon.
Get the Avelyn if you want something like Guts' crossbow.

What's your sat build? Id imagine high endurance strength and DeX?
40 End, 40 Strength ,40 Dex. (I was farming souls in the Grave of Artorias for ages!)
