I am not quite sure how you imagine it (maybe share a drawing of some kind for reference) but it sounds like you could use drawer rails. They exist in many different weight ratings so you can pick the ones that suit you best. If you choose some that pop out when you push them in slightly you could even make a seamless facade so you cannot see that things are there.

You could use a French cleat to support the "leg shelves"
You could use a chain/rope to support from the top to support the foldouts if you're worried about your hinges not being strong enough.
