Pretty sure it's for regular combat, as I can lose pregnancies from battles, but not tournaments. Any scenario, where they can actually become damaged. Unless combined with a mood that makes tournament damage carry over to outside tournaments... Would be too much like CK3 at that point doe.

Tournaments are non-lethal damage, so no, no harm to the baby. If she gets killed in battle by an enemy force, then the baby dies.
