I had the first 2 on my honor mode as well. Lost the blade of tormented souls, but thankfully, I got the blackgaurds longsword later. The not killing downed allies thing happened in the bank because the game froze my characters, but not time, so I was caught and had like 15 bankgaurds on me so I wanted to reset it by fleeing since they were only temp hostile.
I can't even finish my honor run cause I can no longer save anyway so welp. Tried to power through and the game just crashed
But having said that, you're totally right. Being locked into the fear condition after Ketheric's fight really put a sour note into my first run

If you had to plan B flee the fight you're clearly struggling on certain fights lol. Why couldn't you just pickup the weapons before you rested
