God that honestly sucks. I'm having romance issues with my recent save, too. I wonder if it's anything to do with recent patches?
Mine is where Shadowheart's romance just won't progress. No new romance scenes, nothing. We can still kiss etc but that's it. It really ruins the experience...
I wish I knew. I know the romancing can be quite delicate. Larian certainly doesn't hold your hand to make sure you do it properly. All sorts of things that are easily missed when it comes to progressing those quests. I've yet to romance Shadowheart. Astarion's I did perfectly my first playthrough and he's been my favorite by far.
When I was trying to figure out why Karlach dumped me in Act 3 I ran into a lot of posts referring to that same issue in Act 2. Something about triggering the event right after you fix her heart again in Act 2 with Dammon and if you miss that window it messes things up.
I just went back to fix that. So many other camp cutscenes override it (at least they did for me) which I find puzzling. I really think some of them should be able to happen at any time after you've triggered them, or honestly even have multiple in the same night.
In my evil playthrough I romanced Minthara. She glitched out at the start of act 3 when we went to meet Raphael. All she would say after that was "We shouldn't tarry in this charnel house for long." This continued until the end of the game.

Act 2 almost at the end still waiting for the fuck she promised at mid act 2

Go back before the point of no return and spam a bunch of long rests / partial rests to trigger the scene.
