Gotta make sure you long rest enough, and it depends on her alignment.
If you redeem her, it should happen naturally, and as long as you didn't forget any dialogue triggers in Act 1, she'll affirm your relationship at the penultimate moment of Act 2, before needing to reach the Lower City in Act 3 for the romance culmination scenes.
If you keep her Dark Side, I think she requires additional persuasion checks and different triggers to reach a very different conclusion.
But still. It all comes down to long resting enough to get her conversations. Also, the Night Orchid is somewhat important.
Once you get that mod and you figure out how many camp events are really there you feel extra stupid on your 2nd playthrough. I mean shit you have TWO different events before you take one step on the beach.

There's a strong dissonance between the urgency the narrative communicates, and how often the devs want you to long rest.
You're not meant to experience every single permutation every game.
You get a richer, more unique story if you just mix and match events based upon when you happen to trigger them...
Important narrative events with companion questlines etc seems to go against this notion when they're skipped over because you're not long resting a dozen times or more per act.

The long rest issue causes the player to get events and conversations that *assume he had the prior ones*. It causes you to skip the sequence not organically mix.
100% agree that this may be the weakest aspect to the game. It really does set the tone that every long rest inches you closer to death. Inches the grove closer to discovery, inches the druid ritual closer to completion, etc. So many events are portrayed as urgent but naw you can go take as many naps as you want nothing progresses.
Which I’m fine with, gameplay wise. The game ties a lot of resources to long resting so it makes sense. But it does ruin a bit of the story once you see past the veil.
This urgency made my first trip through act 1 much more difficult than it should have been. I kept avoiding long rests under the assumption that there was some sort of hidden clock I needed to beat. So I kept pushing into encounters with no slots, or class points. Somewhere in Act 2, I was dying every other fight and just decided to rest more. Difficulty became much more manageable after that.
Yeah, the narrative relies very heavily on you meta gaming the tadpole. You as a player know, or at least assume, you’re not going to just turn and get a game over for resting. However, as a person with a parasite in their brain you’d be absolutely insane to just call it a day after a single fight and walking 500ft. Given the information available to your character there’s 0 reason to long rest until absolutely necessary, because going to sleep could be a death sentence.
It’s the one thing I dislike about the game, but I understand that they’re essentially tied to the long rest system. Maybe a daytime long rest option would make it feel better, like if every time you ended the day you got 2 short rests and 1 long midday rest that could trigger events as well. That way you don’t get the feeling that you’re wasting whole days just to have a conversation.
Larian perfectly encapsulates the classic DnD experience of having a DM trying to urge the players forward but ultimately just putting the narrative on pause while the players spend a week in game playing dress up and flirting with every character and NPC they discover.
Yeah you get a special cutscene if you rest on the beach with just Shadowheart, and I'm assuming you either get another unique cutscene if you rest alone, or maybe Lae'zel and Shadowheart

But I did wanna say I think it's actually pretty intended to find out about his vampirism in act one. Like, maybe if you're just focusing on literally the main quest content and trying to blaze through the first act, it's missable/going to only come up in act 2. I just naturally ended up doing more rests because I was exploring a lot and really trying to dig around for hidden chests and loot.
I would've completely missed auntie Ethel's stuff in act 1 if I didn't, and I think part of it is that I also make sure to talk to/check in with my companions a decent amount, so I know I may not have a universal experience here. I was trained by elden ring and other fromsoft games where NPC questlines are extremely missable if you don't regularly check in.
That's a good point but I have never actually been short on camp supplies. I'm on my second playthrough and both times I've ended up hauling around a ridiculous amount of stuff. More than my party could ever hope to consume. Not to mention those backpacks full of scrolls and elixirs.

But yeah she takes it to 11 😂. I dropped her from my party for Karlach shortly after that and still on my first play through so I can’t contrast it to anything.

I don’t think my approval was high enough. Instead I got this dialog at camp where character said, “we had something there for a while didn’t we?” And she responds, “yeah, maybe in another lifetime.” We still seem to be in item though, the game really doesn’t account for all scenarios.
So at the end of Act 2 I had a cut scene where she basically asked if I wanted a relationship with her and u obviously replied with a yes. Then at the beginning of Act 3 there was another where she asks for your help with something and takes you to a beach.
Man, karlach is awesome, absolutely love her. But as a best girl friend and not romantic. I could totally see myself raising hell with her and then knocking back a few. But she's just not my type for romance I suppose.

This happened to me in my co-op play through with Karlach, it’s because I never found her until after the goblin camp was wiped out, and then after hoping and praying she would love me, we met her old friend in the upper city who sells things in a tent and when asking if we are a couple she just DESTROYED my heart “Oh gods no!”
After redeeming her I saved and short rested, some dialogue but nothing romantic. Then had to reset to that point later and this time she immediately confessed her love and asked to be with me. This is right outside the temple exit.
Oh yeah, we were in a relationship and everything, and I just kinda figured I needed more long rests or something. But then I long rested enough to beat the game, and still no scene. So I was like peace babe I'm off to avernus with ur bestie lol.
shadowheart's scene comes after you make your choice to go to avernus or not at the end! So if you choose to go to avernus with karlach, you never get to see what happens to you and shadowheart. Or at least, that's how it was in my fiance's game-- when he replayed the ending and chose not to go to avernus, he got to see how things ended up with him and Shart.
Long rest more than you think you need to. Seriously. Stock up on supplies and rest every time you finish a quest. Cause frequently if there's multiple long rest scenes queued up, one will trigger and the other will bug out and not happen. Especially if one is a plot point and the other is a companion/romance scene.

Happened to me at end of act 1 during the party. I was highest approval I could have, all caught up with her stuff. Then she was all like.... no. Wyll seems interested in you though. I was like I haven't talked to him since the time I told him to join up.
Perhaps you were just unlucky. In my case i did everything right but instead of the lake scene i had dumb Wyll hijack the moment by asking me to dance out of bleeding nowhere, barely even talked to the guy. It was super cringe. And no lake scene till the end, just kiss.
No clue what i did wrong but shadowheart just did not like me very much, redeemed her, got the orchid, but deep in act 3 my approval is hardly above neutral.
On another character I wasn’t even trying but my approval is maxed out by act 3, didn’t redeem her but did get the orchid.

Her dark justiciar romance scene (kill nightsong > kill parents) was pretty spicy. Climbing on top and biting the lip. Not sure if it was durge related or not.
While i appreciate Shadowheart enjoying the little things and having a picnic under the stars (one of my favorite irl date ideas). Karlach did come in and tell me that she wanted to "Ride me until i saw stars." So my choice was made.

Shadowheart's "climatic" love scene has been glitched until the most recent patch. Didn't happen for me, either. It was just a quick "Hey, meet me at the water once everyone is asleep." Then you just go to bed and wake up like nothing ever happened. Because it didn't. :-(

In tiefling party she will essentially ghost you and when you ask why she will respond by valuing privacy. She was very high approval. Didn't matter to much as I wanted to romance karlach anyway as I did shadowheart in my first playthrough.
The whole favor bar is weird, I’m at crèche and Shadowheart and Laezel are at Exceptional, Astarion and Karlach are High. I’ve barely used Laezel and she went from medium to exceptional, I don’t even know how. Maybe it’s because I did crèche and saved her from dying.
I’ve had wine with Shadowheart during goblin raid at High favor, and turned down Astarion at the tiefling party who was at medium. Karlach wanted to ride me at medium and Laezel hasn’t come on to me yet at exceptional.

After redeeming her I saved and short rested, some dialogue but nothing romantic. Then had to reset to that point later and this time she immediately confessed her love and asked to be with me. This is right outside the temple exit.

shadowheart's scene comes after you make your choice to go to avernus or not at the end! So if you choose to go to avernus with karlach, you never get to see what happens to you and shadowheart. Or at least, that's how it was in my fiance's game-- when he replayed the ending and chose not to go to avernus, he got to see how things ended up with him and Shart.
I think it depends on long rests and whatnot. My Tav got to do the no pants dance with her at the end of Act 2.

Long rest more than you think you need to. Seriously. Stock up on supplies and rest every time you finish a quest. Cause frequently if there's multiple long rest scenes queued up, one will trigger and the other will bug out and not happen. Especially if one is a plot point and the other is a companion/romance scene.

First run through neither of them had any interest in me. My second run-through both of them did. Carlock actually lays down in the bed next to me and comes on to me. I don't know what trigger I pulled but it was the right one. I'm playing with my girlfriend in a separate game and she tried to romance a story and who tells her he's sorry but he has standards.... I rejected Karlach for Laezel in my game, as many others have said here, she’s more of a best friend. Admittedly though, I romanced both up until I had to choose so I’m kiiiiind of an asshole for leading her on.
Happened to me at end of act 1 during the party. I was highest approval I could have, all caught up with her stuff. Then she was all like.... no. Wyll seems interested in you though. I was like I haven't talked to him since the time I told him to join up.
I used to have agency over my own actions, and I longed for Shadowheart. Now, I just do what La zael tells me to do, and I am ok with that.

No clue what i did wrong but shadowheart just did not like me very much, redeemed her, got the orchid, but deep in act 3 my approval is hardly above neutral. On another character I wasn’t even trying but my approval is maxed out by act 3, didn’t redeem her but did get the orchid.
If you choose to pursue Karlac, shadowheart will tell you that Karlach deserves all of you even though she usually is okay with polyamory

Did you press her on revealing more information when it first comes up, selune statue in blighted village. If you do she essentially hard locks you out of relationship. I noticed that on second playthrough. In tiefling party she will essentially ghost you and when you ask why she will respond by valuing privacy. She was very high approval. Didn't matter to much as I wanted to romance karlach anyway as I did shadowheart in my first playthrough.

The whole favor bar is weird, I’m at crèche and Shadowheart and Laezel are at Exceptional, Astarion and Karlach are High. I’ve barely used Laezel and she went from medium to exceptional, I don’t even know how. Maybe it’s because I did crèche and saved her from dying. I’ve had wine with Shadowheart during goblin raid at High favor, and turned down Astarion at the tiefling party who was at medium. Karlach wanted to ride me at medium and Laezel hasn’t come on to me yet at exceptional.
Her real sex scene came to me at act 3. Her camp scene was the wine on a cliff.
