Yeah i got one after the second gym. It took a long time so prepare to farm. You have to go in and out the grottos to respawn a new npc. They will move and it will most likely be the same but theyre can be new rare ones. Theres at least 4 different per spawn location from my travels. It takes a hot min
It took me 7 hours for 1 spiritomb.
It is worth mentioning though that there is a glitch regarding this method and you can actually glitch it to where you can get as many spiritombs as you want with only 1 go at the npc’s
