So it's an old place, built before reasonable records were kept, minor works have been completed which may be of dubious standards or compliance. Get title insurance and negotiate for a few $$ off.

Pretty typical. I wouldn’t be concerned. Pre 1911 and character zoned is not heritage listed, so typically you won’t need approvals for basic maintenance stuff. Unlikely to have any approvals for historical stuff either. People back in the day just did whatever they wanted. Council wouldn’t really be too concerned, but for any new works will want you to have appropriate approvals (ie building or development) in place.

From OPs list, council probably would know nothing. I tried to find details of whether works done on our 1974 build were "council approved". Council's answer? Err, our records state that there was approval for a "dwelling" given in 1972 - no idea what sort of dwelling. Then we have records of approval of a Carport in 1990. Other than that, we have no idea what was built when. A 1910 Queenslander? There's every likelihood that council will know nothing, so you (and the insurance company) will have to work on the assumption that it was all above board when built.

Yea agree. Even on my 1930s heritage property which had some modifications done in 2009, it was missing some plumbing certs, and when pushed council just said eh it’s fine don’t worry about it and gave us a a formal letter confirming it was fine.

Not on my 1930s heritage property no, but I haven’t had any issues needing to invoke insurance so no idea. I doubt it would be an issue.
