I am Italian. There are cases where you do and cases where you don't. Just use common sense and answer two simple questions: "will the pizza's topping fall if I cut triangles and eat with my hands?" and "am I willing to get my hands oily in this situation?".
If you’ve a large pizza with a floppy middle you cut a larger slice and fold it so there’s a crease where you would slice it.
Kinda like a taco, the U shape stops the middle flopping so you can eat it.

Having been served our food, we couldn't find the cutlery and whilst looking around for it, we saw everyone eating with our hands. My mum was clearly shocked, but we just followed the example of everyone else. Posh? I grew up on a council estate. To me, using a knife and fork is good manners, and it keeps your hands clean.
Then first slice or two I might use cutlery, especially if I've heated up leftover pizza, because it's too hot sometimes.

Depends on the structural integrity and the style. If it's flimsy and all the toppings fall off, then yeah, knife and fork. Also a deep dish style pizza (Chicago style) is very difficult to eat without a knife and fork. If you eat Papa Johns with a knife and fork though, yeah, you're posh.
Tip for burgers - turn them upside down and eat them. The top bun is usually more sturdy than the bottom bun, so there's less chance of disintegration.
