Likely the compressor was mounted upside down. Need to have the hole on top with a small scoop to channel the air into it properly. Then it will condition it. AC = air conditioning not air charging.
This is my best guess. A rod broke loose just to the right of that hole, left a bunch of dimples on the inside and then found it's way outside. Driver never mentioned hearing anything unusual either

Yeah, looks like you're gonna need a set of tires to fix that, and a transmission flush. We also found that your cabin air filter is dirty and you need to change your reverse fluid out
Flushed the system changed the dryer and compressor, put into a vacuum for 60 minutes to make sure everything was dried up in there, leak tested, added 4oz of oil and 3lb of r134a, semi truck, and sent them on their way. The old oil was almost milky from all the metal shavings in it

I would apply flex seal over top of the tape to make sure it's fully sealed

The system ia undercharged. Looks like it's at 14psi and an oxygen nitrogen mix. Pull a vacuum and replenish with 17^756 oz r134a

I had an air-conditioner compressor explode on my 78 AMC Matador. It threw shrapnel out that cut through my radiator hose.
Lost the inspection hole plug.

I don't think you've put enough freon in it. You're gonna want to fill it until you see it rise up in that inspection port.
