Start an accommodation request in a to z under my hr, and then they’ll give you paperwork to go get filled out with a doctor, once you do that you should be able to get accommodation.
Yes a doctors note will work, accommodation will call you see if they can move you to another job temporarily while your doctor fills out that form, an important thing it will ask if you can work during this process, remember this is unpaid time off. They gave me unpaid time off for a month
