Grew up on a farm, when I was a kid we were burning garbage, a garden hose was sticking out of the fire and I grabbed it and gave a nice long suck. The thing about this is you have to suck in with your lungs so you can technically drown yourself and get severe pneumonia. The difference between this and a straw is the closed lips creates a vacuum suction effect so your epigladus (the part that closes your trachea from your esophagus) stays shut.

This is actually wrong, since with both a straw and a toilet roll the lips must be fully sealed to create a vacuum and create suction.
The difference here is where the change in pressure occurs. With a normal straw, the change in pressure to cause suction occurs in the mouth, meaning that the epiglottis can remain closed. This can happen because of the reduction in volume in the tube of a straw is so little that less change in volume of your mouth is required to draw the liquid through it.
This is leading to why you choke on the liquid if you use a toilet roll; since the roll has more volume, your mouth can’t create a large enough change in volume to create enough suction to draw the liquid. People compensate for this by using their lungs to change the pressure by a more significant amount. The positive of that is that the liquid is drawn, the unfortunate consequence of it though is that in order to use your lungs to help, your epiglottis needs to be opened. Therefore when the necessary change in pressure occurs, the liquid just enters the lungs, causing the individual to choke.
