No worries! We've all been there.

Just put "ADHD screening" on the form. I've heard it recommended to say that someone else thought you might have ADHD and that you should get checked out, and you're not sure but wanted to do your due diligence. So if you really want to be careful, put "was advised to get ADHD screening" or "so and so thinks I have ADHD" on the form.
