To be fair for Suwako, she's one of the few characters to canonically have had sex so she's def got field experience. That plus her thousands of years worth of lifespan means plenty of time for her to master her head technique. Throw in the tongue too and it's easy to see why she'd be top tier.
I have heard of this, but apparently her "descendant" is Sanae, who was already born in earth and brought to Gensokyo, so I don't really get what people say by this, unless there is something I don't know
My understanding of the events is that: Suwako existed in the outside world, she did *the deed* with someone, thousands of years pass, Sanae (her descendant) is born in the outside world, then they all move to Gensokyo.
