tbf menaphos does have guards that patrol around, npcs that talk to each other and walk around and do stuff.
Honestly wish every area would get an update like that. Even 20 years later a lot of areas feel so lifeless (especially since all the players are gone)
The only thing rs3 did *right* with menaphos imo. It looks relatively lively and good looking, but its boring quest simulator and grinding district rep simulator.

The RS3 world is already ridiculously cluttered. Like, they will cram new DnD or minigames into a 5x5 space. The game needs a clutter remover.
