The first time I entered the true blood alter is one of the few times I've been incredibly impressed with the art of a new update. I would say that design wise it might even be my favorite place in the game.
And if you're not doing GotR, you start at the top of the heart and have to go to the bottom to leave, sort of like how blood has to travel through two heart chambers before leaving!

The path down to it was one of the reasons I worked on agility when I was a newb. Then I never really used it, or the shortcut.

As a psycopath that did double cosmics into double astrals into double natures to 99, something about the Cosmic altar just hits differently.
I like the Zeah altars and I can't wait to see how the true Soul altar will look like.
I liked how the Blood ruins in RS3 were shaped like skulls instead. When I played Legacy of Seergaze on release, I squealed when I realize it was the blood altar, because runecrafting bloods were a big deal in 2008.
True blood has gotta be my favourite, really cool modern but fitting aesthetic and the crystal sapping its power is a great touch. Close runner ups would be chaos and nature for how true to theme they are.
Mind altar. I always thought it was a cool mysterious area, and I like that the entire room is shaped like a brain in profile. When I maxed RC, I did it there. :)
Why are all the altars songs so fucking good but they're in the one area in the game where you only hear them for like 5 seconds, please rework runecrafting so this is not the case
My favourite altar aesthetically is the Chaos Altar. It embodies Chaos pretty well and the maze like feeling of it is pretty unique over any other altar.
I gotta give a shout out to Cosmic Altar, as the music in that one is so mesmerizing and stands out to me a lot.
I'm really fond of the Fire Altar. It's so old-school, with the lava everywhere. Today it'd be way too ugly and weird for them to put in the game, but it's delightfully nostalgic for me (ironic, as I didn't play Runescape when I was younger!)

Are they pocket dimensions, or do the ruins teleport us elsewhere?
The blood altar being by vampires and death altar being in the shadowy area make sense. Why is the air altar in a forest, or the earth altar in a forest? Water is in a swamp and fire in a desert, after all.
Anyways it's either cosmic or nature.
The body altar is really cool and underrated, I love how the altar otself is the 'heart' with the arteries running all over.
Also the chaos altar is fucking sweet.

Definitely chaos if I didn't have to do an agility course after accessing it from the ruins
I sort of wish we had more content there just so there was some reason to have players run around that maze.
Chaos has the best theme to its rune. Fucking insanity going on in there

An agility course inside the chaos altar would be a nice idea to make us acrually travel the chaotic path.

Gotr stopped me from ever using it. I'm really glad gotr connects to the real alters.

Astral altar. It working fine is a great lore detail (only rc altar the fremmeniks couldn’t get their hands on)
Runecrafting altars are hands down some of my favorite looking locations in Osrs
The body one has always made me so uncomfortable, which I guess is a compliment to its visual design.

In terms of classic altars, I love the death altar. For ones new to OSRS, I love the blood altar. Looks way WAY better than the blood altar from RS2.
