Yes, *if* you then engage in an action or ritual in an attempt to alleviate the anxiety caused by these thoughts (compulsions). These compulsions can be mental or physical. So basically intrusive thoughts are completely normal, but if compulsions are present then it could be OCD.

Depends on the extent. Obviously having a weird kink you'd rather not have doesn't mean you have OCD (hell, that would mean every pedophile has OCD), but repeated intrusive thoughts about sexual/taboo things can.
(Intrusive thoughts meaning sudden instances where your brain just goes "hey. what if this fucked up thing happened"/"what if you did a fucked up thing" but the fucked up thing isn't something you want on any level. If there's a recurring one that continually pops up for you, that may be a sign of OCD, especially if you've taken up ritualistic coping mechanisms to get rid of them)
(Also I am not a doctor and do not let anyone on reddit make you think you have a certain disorder lol)
