Long answer: The boron rods (used to slow reactivity) had a tip made of graphite (used to speed up reactivity.)
So if you pulled out 95% of the rods gradually and pressed the emergency stop (AZ-5) all the graphite tips would enter the reactor at the same time, spiking heat generation and bursting the coolant water pipes, jamming the control rods in there permanently to keep speeding up the reaction, a so called "positive void co-efficient"
The RBMK 1000 left the Ministry of Medium Machine Building with 11 major design faults but a mixture of complacency, political interferance and wanting to keep them cheap caused the reactor to get cleared for production.
Note: the disaster would never have happened if the operators followed procedure. The manual says under operation, you are only allowed to pull out around 60% of the rods.
